
Notes For Lab Staff
This is where you can request items that are not on the prescription form, including descriptions of pads and additions.
We will make every effort to diligently follow any instructions provided in the "notes for lab staff" section. We strongly encourage podiatrists to utilise the available selections in the script to minimise any potential confusion. It is important to note that this section is not intended for clinical patient notes, and any information included here will be regarded as an instruction or further explanation for the manufacturing process.
Example Uses:
If you have marked the software drawing, scan or cast, please place the reasons why and mention the colour you have used, if any. We ask practitioners to mark each instruction with a 1, 2, 3 to differentiate each instruction.
When requesting a specific shape or size for the orthotic to be cut out (templates or shoe size), this information needs to be placed in 'Notes For Lab Staff'.
We stock Revere sandal and shoe templates. We require all the information on the bottom of the innersole provided in the sandal. This includes a European size (Numbers ranging is 35 to 48) and a long code with both numbers and letters (example, UAS-003-10). Please refer to our price list for Revere template fees.
We carry a range of US size templates at the laboratory, which will be used if a US size is placed in the Notes For Lab Staff section. There is no universal shoe size guide; therefore, the US size your patient wears might be different to the US size we have in our template range. We suggest supplying a template with measurements if you require specific measurements.
